  This is an online e-log book to discuss our patient's de-identified health data shared after taking his/her/guardian's informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. A 60 year old female who is daily wage labourer by occupation came with the chief complaint of 1. Lower back pain  2. Facial puffiness 3. Pedal edema History of present illness : - Patient was apparently asymptomatic 4 months back then she developed fever and backache for which she was treated symptomatically.   Final internal assessment of batch 2018 BDS based on overall FORMATIVE AND SUMMATIVE ASSESSMENTS 17 September 2021 DEPARTMENT OF GENERAL MEDICINE Final internal assessment of batch 2018 BDS based on overall FORMATIVE AND SUMMATIVE ASSESSMENTS This final internal assessment is based on formative and summative assessments of the learning competencies demonstrated by the 2018 BDS batch posted in the department of Medicine. The formative assessment is from each UG student’s  logged competency around patients evaluated regularly from date of joining. The formative assessment has been divided into non-verbal which assesses their competency in procedural domains including empathic communication and body language and verbal which assesses their ability to communicate their work through their day to day E logs.
