  This is an online e-log book to discuss our patient's de-identified health data shared after taking his/her/guardian's informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs.  Date of admission : 20 December 2021 A 35 year old male patient who is a daily wage labourer has come to the OPD with the chief complaints of dizziness and vomitings since 2 days. History of present illness : Normal routine of the patient : The patient gets up in the morning by 5/6 AM, has tea at 8 AM, eats rice at 10 AM and goes to the work. Abdominal distension since 10 days 2. Scrotal swelling since 10 days 3.
